Kentucky Code § KRS 213.096

Combination birth-death or stillbirth certificate -- Completion of form

(1) Each fetal death of twenty (20) completed weeks' gestation or more, calculated from the date last normal menstrual period began to the date of delivery or in which the fetus weighs three hundred fifty (350) grams or more, which occurs in the Commonwealth, shall be reported on a combination birth-death or stillbirth certificate in accordance with applicable provisions of KRS 213.046 and KRS 213.076. If the fetal death occurs in a hospital, the person in charge of the institution or the person's designated representative shall complete the stillbirth certificate, obtain the medical certification, and file the certificate with the state registrar. (2) The name of the father shall be entered on the stillbirth certificate in accordance with the provisions of KRS 213.046. (3) All abortions shall be reported in the manner prescribed in KRS 213.101 and shall not be reported as stillbirths. (4) If requested by the patient to whom an abortion is provided, the person in charge of the institution or the person's designated representative shall complete the form created by the cabinet under subsection (3) of this section, obtain the medical certification, and file the certificate with the state registrar.
