Kentucky Code § KRS 116.047

Forgery, alteration, destruction or failure to return completed registration form

Any individual who forges or fraudulently alters a form completed by the prospective registrant, who willfully destroys a completed registration form or who willfully f ails to return a completed form to the county clerk for official registration by the county clerk shall be subject to the penalties described for Class D felonies.
