The application for appointment of a nonresident executor or trustee shall include the name and address of the nonresident executor or trustee, and the name and address of the resident executor or trustee, ifany, to be appointed. It shall be accompanied by a certificate of the clerk of the foreign court granting the original letters or other authority conferring the power upon the nonresident executor or trustee to act as such. The application shall also state the cause for the appointment of the nonresident executor or trustee to act as the sole executor or trustee, ifsuch appointment is desired. When the will has not been admitted to probate in any other state, the application shall include the name and address of the executor or trustee, ifany, named in the will of the nonresident, and of the resident executor or trustee to be appointed.
