An adoption petition shall have attached to itthe following: 1. A certified copy of the birth certificate showing parentage of the person to be adopted or,if such certificate is not available, a verified birth record. The department shall provide a certified copy of a birth certificate or a verified birth record, as applicable, to the person adopting a child when the department is the guardian of the child. 2. A copy of anyorder terminating parentalrights with respect to the personto be adopted. 3. Ifparental rights were terminated pursuant to chapter 232, a copy of any court orders concerning whether ongoing contact between siblings not placed with the person to be adopted is in the best interest of each sibling. 4. Any written consent and verified statement required under section 600.7, except the consent required under subsection 1, paragraph “d”, of that section. 5. Any preplacement investigation report that has been prepared at the time of filing pursuant to section 600.8. 6. In the case of a standby adoption as defined in section 600.14A, a form completed by the terminally illparent consenting to termination of parental rights and adoption of the child by a person or persons specified in the consent form, effective at a future date when the terminally illparent of the child has died or requests that a final adoption decree be issued.
