A person’s license to practice medicine and surgery or osteopathic medicine and surgery shall be deemed relinquished ifthe person fails to apply for renewal or reinstatement of the license within five years after itsexpiration. A license shall not be reinstated, reissued, or restored once it isrelinquished. The person may apply for a new license pursuant to section 148.3 and applicable administrative rules.
