A person who violates apermanent injunction issued pursuant to section 103.37 or presents or attempts to fileas the person’s own the certificateof licensure of another, orwho gives false or forged evidence of any kind to the board in obtaining a certificate of licensure, or who falsely impersonates another practitioner of like or different name, or who uses or attempts to use a revoked certificate of licensure, is guilty of a fraudulent practice under chapter 714.
