Sec. 1. (a) Each application for a permit or driver's license under this chapter must: (1) be made upon the approved form for the application furnished by the bureau; (2) include a signed affidavit in which the applicant swears or affirms that the information set forth in the application by the applicant is correct; and (3) include a voter registration form as provided in IC 3-7-14 and 52 U.S.C. 20504(c)(1). However, an online application does not have to include a voter registration form under subdivision (3).       (b) The Indiana election commission may prescribe a voter registration form for use under subsection (a) that is a separate document from the remaining portions of the application described in subsection (a)(1) and (a)(2) if the voter registration form remains a part of the application, as required under 52 U.S.C. 20504(c)(1).
