Application forms for certificates to operate motor vehicles; fees

Sec. 40. (a) All applications under this chapter for a certificate to operate motor vehicles, intrastate or interstate, shall be made on forms prescribed by the department.       (b) All applications for a certificate to operate motor vehicles intrastate on the public highways shall be accompanied by a filing fee of one hundred dollars ($100). Each petition for reinstatement of a certificate to operate motor vehicles intrastate, on the highways of this state, shall be accompanied by a filing fee of fifty dollars ($50). A common carrier reinstatement shall only occur within two (2) years of the voluntarily ceased operations date provided to the department by the carrier.       (c) All applications for an emergency temporary certificate to operate motor vehicles on the highways of this state in intrastate commerce, shall be accompanied by a filing fee of one hundred dollars ($100).       (d) All applications for a change in the name of the holder of a certificate, which change of name does not involve a change in the ownership of the operating rights of the certificate holder, shall be made by verified petition to the department, and the applications shall be accompanied by a filing fee of twenty-five dollars ($25).       (e) Each application for alteration or change of a certificate to operate motor vehicles intrastate, on the public highways, shall be accompanied by a filing fee of fifty dollars ($50).
