Notice of lien; contents; signature; verification

Sec. 12. A notice of lien recorded under this chapter must: (1) state: (A) the name of the claimant; (B) the name of the owner of the commercial real estate upon which the lien is claimed; (C) a legal description of the commercial real estate upon which the lien is claimed; (D) the amount for which the lien is claimed; and (E) the license number of the broker company's license under IC 25-34.1 ; (2) contain a statement that the information contained in the notice is true and accurate to the knowledge of the signatory; (3) be signed by the managing broker responsible for the actions of the broker company or by a person authorized to sign on behalf of the managing broker; and (4) be verified.
