Indiana Code § 31-9-2-99.3

"Provider"

Sec. 99.3. (a) "Provider", for purposes of IC 31-28-2 and IC 31-28-3 , means an individual, a partnership, a corporation, or a governmental entity that is enrolled in the Medicaid program under rules adopted under IC 4-22-2 by the office of Medicaid policy and planning.       (b) "Provider", for purposes of IC 31-28-1 , has the meaning set forth in IC 31-28-1-2 .       (c) "Provider", for purposes of IC 31-27 , means a person who operates a child caring institution, foster family home, group home, or child placing agency under IC 31-27 .
