Sec. 8. (a) This section applies to an individual who: (1) is leaving foster care because the individual is at least eighteen (18) years of age; and (2) has been in foster care for at least six (6) months. (b) Before an individual described in subsection (a) leaves foster care, the probation officer shall provide to the individual the following documents that are applicable to the individual: (1) An official or certified copy of the individual's United States birth certificate. (2) A Social Security card issued for the individual by the Social Security Administration. (3) Insurance records for the individual. (4) A copy of the individual's medical records. (5) The individual's driver's license or identification card issued by the state. (6) Information concerning the individual's enrollment in the Medicaid program. (7) A foster care verification form.
