Sec. 3. The local office of the county in which a foster child resides shall maintain a health summary record for the foster child. The provider that has provided ongoing care to the child shall complete the record. The record must include the following: (1) A summary of health care provided to the child. (2) Recommendations for future health care needs of the child.
