Sec. 4. The confidential intermediary shall: (1) make complete and reasonable efforts to locate the medical, identifying, or nonidentifying information; (2) attempt to locate any person necessary to obtain the medical, identifying, or nonidentifying information; (3) inform the person contacted of the medical or other need set forth by the petitioner; and (4) obtain the needed medical, identifying, or nonidentifying information.
