Voluntary transmission of medical information; storage; affirmation

Sec. 3. (a) Any person may voluntarily transmit medical information to the state registrar for inclusion with the medical history.       (b) The state registrar shall store all information received under this section in a manner that makes the information readily recoverable.       (c) Any transmission of voluntary information must include an affirmation by the person that: (1) the information is true; or (2) the person believes the information is true.
