Indiana Code § 3-5-7-6.5

Candidacy document design; separate fields for each separate candidate name designation

Sec. 6.5. (a) As used in this section, "candidate" includes a nominee.       (b) The election division shall design all candidacy documents so that the form of the document enables the candidate to insert in a separate field of the document each of the separate designations that a candidate is permitted to use under this chapter.
