Indiana Code § 27-8-4.9-3

"Plan sponsor"

Sec. 3. (a) As used in this chapter, "plan sponsor" means a person that establishes, adopts, or maintains a health benefit plan that covers individuals in Indiana.       (b) The term includes a health benefit plan established, adopted, or maintained: (1) by an employer; (2) jointly by an employer and one (1) or more employee organizations; or (3) by an association, committee, joint board of trustees, or any similar group of representatives.       (c) The term does not include an entity regulated under this title.
