Sec. 1. Each health maintenance organization subject to this article shall pay to the commissioner for deposit into the department of insurance fund established by IC 27-1-3-28 the following fees: (1) Three hundred fifty dollars ($350) for filing: (A) an application for a certificate of authority; or (B) an application for an amendment to a certificate of authority. (2) Fifty dollars ($50) for filing each annual report.
