Articles of incorporation; form; execution

Sec. 6. The form of the articles of incorporation shall be prescribed and furnished by the department. The articles of incorporation shall be: (1) prepared and signed in triplicate originals by all of the incorporators, or, in the case of a redomestication under IC 27-1-6.5 , by the corporate officers if the original incorporators are no longer available; (2) acknowledged by at least three (3) of the incorporators or corporate officers before a notary public; and (3) presented in triplicate originals to the department at the office of the department.
