Sec. 6. If the department receives information under section 4 of this chapter from a health care provider or health plan, the department shall keep the following information of the individual who submitted the information confidential: (1) Name. (2) Address. (3) Telephone number. (4) Electronic mail address. (5) Personal health information. (6) Any other information that could identify the individual.
