Indiana Code § 27-1-37.5-1.6

"Authorization"

Sec. 1.6. As used in this chapter, "authorization" means a determination by a utilization review entity that: (1) a health care service: (A) has been reviewed; and (B) based on the information provided, satisfies the utilization review entity's requirements for medical necessity; and (2) payment will be made for the health care service.
