Sec. 6.1. An applicant for a license as a physical therapist must do the following: (1) Complete the application process, including payment of fees. (2) Submit proof of graduation from a professional physical therapist education program accredited by a national accreditation agency approved by the board. (3) Pass an examination approved by the board. (4) Submit to a national criminal history background check under IC 25-1-1.1-4 . (5) Meet any other requirements established by board rule.
