Sec. 4. (a) Before March 31 every year, a certified direct entry midwife shall provide an annual report to the board regarding each birth the previous year that the certified direct entry midwife assisted. A report must summarize the following on a form prescribed by the board: (1) Vital statistics. (2) Scope of care. (3) Transport information. (4) Physician referral. (b) A certified direct entry midwife may not reveal the identity of the clients referred to in a report under subsection (a). (c) The board shall compile the data from the reports collected under subsection (a) and submit the data to the Indiana department of health.
