Expiration and renewal of license

Sec. 5. (a) Subject to IC 25-1-2-6 (e), a license issued by the board expires on the date established by the agency under IC 25-1-5-4 in each even-numbered year.       (b) To renew a license, an acupuncturist must: (1) pay a renewal fee not later than the expiration date of the license; and (2) submit proof of a current active certificate in acupuncture by the National Certification Commission for Acupuncture and Oriental Medicine.       (c) If an individual fails to pay a renewal fee on or before the expiration date of a license, the license becomes invalid without further action by the board.       (d) If an individual holds a license that has been invalid for not more than three (3) years, the board shall reinstate the license if the individual meets the requirements of IC 25-1-8-6 (c).       (e) If more than three (3) years have elapsed since the date a license expired, the individual who holds the license may seek reinstatement of the license by satisfying the requirements for reinstatement under IC 25-1-8-6 (d).
