Revocation of license for failure to pay renewal fee; relicensing

Sec. 4. (a) If a person fails to pay the renewal fee for two (2) consecutive years, that person's license shall be revoked and may not be renewed.       (b) If a person whose license has been revoked under subsection (a) desires to be relicensed, the person: (1) must submit an application to the board; and (2) shall be considered as a new applicant.
