Sec. 4. An individual must meet the following requirements in order to qualify as an eligible patient under this chapter: (1) Has been diagnosed with a life threatening or severely debilitating disease, as attested by the individual's physician. (2) Has considered other treatment options currently approved by the United States Food and Drug Administration. (3) Has received a recommendation from the individual's physician for an individualized investigational treatment based on analysis of the patient's genomic sequence, human chromosomes, deoxyribonucleic acid, ribonucleic acid, genes, gene products, or metabolites. (4) Has given written informed consent as set forth in section 5 of this chapter for the use of the individualized investigational treatment. (5) Has documentation from the individual's physician that the individual meets the requirements of this section.
