Sec. 4. (a) Before issuing a certificate of free sale, a business shall provide the following to the state department: (1) Proof of registration with the Indiana secretary of state. (2) The most recent inspection report showing the business is in good standing. (3) A completed application. (4) The fee for the certificate of free sale.       (b) The state department shall charge the following fees for issuing a certificate of free sale: (1) For each original certificate, a fee of twenty-five dollars ($25). (2) For each additional copy, a fee of five dollars ($5).
