Sec. 12. (a) A practitioner shall require a semen donor to provide the following information: (1) Name. (2) Address. (3) Date of birth. (b) A practitioner shall request a semen donor to provide the semen donor's Social Security number. [Pre-1993 Recodification Citation: 16-8-7.5-13.]
