Birth certificate requests; permanent records

Sec. 7. (a) Both the state registrar and the local health officer shall maintain a permanent record, accompanying the birth records, that contains for each request for a certified copy of a birth certificate the following: (1) The date of the request. (2) The name and address of the person making the request.       (b) The permanent record for each request required under this section may be maintained in the Indiana birth registration system (IBRS).
