Issuance of DNR order

Sec. 12. An out of hospital DNR order: (1) may be issued only by the declarant's attending physician, advanced practice registered nurse, or physician assistant; and (2) may be issued only if both of the following apply: (A) The attending physician, advanced practice registered nurse, or physician assistant has determined the patient is a qualified person. (B) The patient has executed an out of hospital DNR declaration under section 11 of this chapter.
