Indiana Code § 16-21-19-4

Failure to submit forms; fine

Sec. 4. If a nonprofit hospital fails to submit the forms required under section 2 of this chapter before October 1 of any year, the state department shall fine the nonprofit hospital system ten thousand dollars ($10,000) per day for which the forms are past due.
