Sec. 1. (a) An application for a license under this article must be made on forms prescribed by the board. The board may adopt a rule that requires: (1) the information on the application to be subscribed and sworn to or affirmed before a notary public by the applicant or the applicant's authorized representative; and (2) that the affidavit is a component of the application. (b) The application must include the information required by this article. The board shall use the information from the application and other information as the board considers pertinent to determine whether a license should be issued, suspended, revoked, or denied.
