Information to be included in checkoff refund form

Sec. 34. The checkoff refund form must: (1) contain the address and fax number of the location to which the assessment refund form may be sent; (2) contain information concerning procedures to claim an assessment refund; and (3) contain any other information determined necessary by the council.
