Sec. 17. If the commissioner determines that an applicant has not successfully completed a voluntary remediation work plan approved under this chapter, the commissioner shall notify the applicant of this determination under IC 4-21.5 . [Pre-1996 Recodification Citation: 13-7-8.9-17(c).]
