Sec. 1. When assistance is granted to a dependent child under IC 12-14-1 through IC 12-14-8 , the award made must be entered on a written notice prescribed by the division that designates the following: (1) The name and residence of the recipient. (2) The amount of the award. (3) The date when the assistance is to begin. (4) Any other fact required by the division. [Pre-1992 Revision Citation: 12-1-7-8 part.]
