A person shall be qualified for licensure as an occupational

Sec. 8. 

A person shall be qualified for licensure as an occupational
therapist if that person:

 
 
(1) has applied in writing in form and substance to 
 
the Department;

 
 
(2) (blank);

 
 
(3) has completed an occupational therapy program 
 
leading to a masters or doctoral degree, or its equivalent, approved by the Department; and

 
 
(4) has successfully completed the examination 
 
authorized by the Department within the past 5 years.
