Application for original license.
Applications for original licenses shall be made to the Department on forms prescribed by the Department and accompanied by the required fee, which is not refundable. If an applicant fails to obtain a license under this Act within 3 years after filing the application, the application shall be denied. The applicant may make a new application, which shall be accompanied by the required nonrefundable fee.
