Idaho Code § 67-2613

Issuance of licenses -- Loss of license -- Issuance of duplicate -- Fee.

All certificates, licenses and authorities shall be issued by the bureau of occupational licenses in the name of such bureau, with the seal thereof attached: provided that if any certificate, license or authority issued by such bureau shall be lost or destroyed, the bureau is hereby authorized on application of the owner thereof, to issue a certified copy or a duplicate under the seal of such bureau; the applicant asking for such duplicate or certified copy, shall accompany the application with an affidavit setting forth the facts showing that the original has been lost or destroyed; that for such certified copy or duplicate, the bureau shall charge a fee of ten dollars ($10.00), which fee shall be by the bureau handled as other fees collected for licenses.
