The certificate of acknowledgment of an instrument which is executed by a person in his own name as trustee or as executor, administrator, guardian, sheriff, receiver or other official or representative capacity, shall be substantially in the following form: State of Idaho, county of ...., ss. On this .... day of ...., in the year ...., before me (here insert the name and quality of the officer) personally appeared ...., known or identified to me (or proved to me on the oath of ....), to be the person whose name is subscribed to the within instrument as (here insert the official or representative capacity in which the instrument is executed) and acknowledged to me that he (or they) executed the same as such (here insert again the official or representative capacity in which the instrument is executed).
