The certificate of acknowledgment of an instrument executed by a limited liability company must be substantially in the following form: State of Idaho, county of .............., ss. On this ......... day of ................., in the year ...., before me (here insert the name and quality of the officer), personally appeared ..........., known or identified to me (or proved to me on the oath of .............), to be the manager or a member of the limited liability company that executed the instrument or the person who executed the instrument on behalf of said limited liability company and acknowledged to me that such limited liability company executed the same.
