Each applicant for a license shall: (1) Make application to the board on forms authorized by the board and furnished thereby, such application to contain proof under oath by the applicant of the particular qualifications of the applicant and contain the names and addresses of three (3) character references. (2) Furnish to said board a passport photograph of the applicant taken within the year preceding the filing of the application, together with a description of the applicant. (3) Pay to the board the required fee. (4) Provide such documentation and information to establish that the applicant meets the requirements for the license sought.
