(1) Each course provider's certification issued by the commission shall be for a term of two (2) years. The exact expiration date will be shown on the provider certificate. (2) In order to maintain certification, each provider shall: (a) Return a properly completed renewal application on a form provided by the commission, along with all necessary attachments and renewal fees to the commission office prior to the expiration date for commission approval; and (b) Certify that its designated director or person in charge has, within the past two (2) years, attended a commission-approved provider training. (3) Recertification is not effective until the commission has formally approved the application for renewal. (4) Failure to obtain approved renewal of certification prior to its expiration date will result in no credit being given for courses not yet successfully completed by the expiration date.
