(a) The practitioner dispensing a controlled substance listed in schedule II shall affix to the package a label showing date of dispensing, the dispenser's name and address, the serial number of the prescription if applicable, the name of the patient, the name of the prescribing practitioner, and directions for use and cautionary statements, if any, contained in such prescription as required by law. (b) The practitioner dispensing controlled substances listed in schedule III or IV shall affix to the package a label showing the dispenser's name and address, the serial number if applicable, and date of initial dispensing, the name of the patient, the name of the practitioner issuing the prescription, and directions for use and cautionary statements, if any, contained in such prescription as required by law. (c) The practitioner dispensing a controlled substance listed in schedule V pursuant to a prescription shall affix to the package a label showing the dispenser's name and address, the serial number if applicable, and the date of dispensing, the name of the patient, the name of the practitioner issuing the prescription, the directions for use and cautionary statements, if any, contained in such prescription as required by law.
