(1) The secretary of state shall prescribe and furnish on request forms for: (a) A foreign corporation's application for a certificate of authority, or amended certificate, to transact business in this state; (b) A foreign corporation's application for a certificate of withdrawal; and (c) The annual report. (2) The secretary of state may prescribe and furnish on request forms for other documents required or permitted to be filed by this chapter but their use is not mandatory.
