Operation permit required for each public swimming pool

A separate application for an operation permit must be submitted for each public swimming pool. The owner or operator shall apply annually to the county board of health for an operator's permit. A form must be obtained from the county board of health to provide: (1) The owner's name, address, and telephone number; (2) The operator's name, address, and telephone number; (3) The street address of the public swimming pool; (4) The physical location of the public swimming pool; (5) The type of public swimming pool; (6) The construction date, if applicable; (7) The proposed operating dates; (8) The type of disinfection; and (9) The signature of the owner or a designated representative of the owner.
