Biennial qualification renewal

A qualification must be renewed every 2 years. A qualification must be renewed by furnishing such information as the commission requires. A complete biennial renewal of qualification must include a declaration under penalty of perjury, signed by the executive officer or managing member of the qualified limited service affiliate seeking renewal, declaring that the information submitted for the purposes of renewal is true and correct to the best of his or her knowledge, and confirming or providing all of the following: (1) That the qualified limited service affiliate is in compliance with this part. (2) The physical location of the principal place of business of the qualified limited service affiliate. (3) The telephone number of the qualified limited service affiliate. (4) A list of the qualified limited service affiliate’s current directors, executive officers, principal shareholder, managers, managing members, or equivalent positions. (5) Any updates or changes in information which were not previously provided either in the initial qualification or in subsequent qualification renewals or which were not previously disclosed to the office.
