(1) At least 90 days before the end of a licensure cycle, the department shall: (a) Forward a licensure renewal notification to an active or inactive licensee at the licensee’s last known address of record with the department. (b) Forward a notice of pending cancellation of licensure to a delinquent status licensee at the licensee’s last known address of record with the department. (2) Each licensure renewal notification and each notice of pending cancellation of licensure must state conspicuously that a licensee who remains on inactive status for more than two consecutive biennial licensure cycles and who wishes to reactivate the license may be required to demonstrate the competency to resume active practice by sitting for a special purpose examination or by completing other reactivation requirements, as defined by rule of the board. (3) Notwithstanding any other law, a licensure renewal notification required to be sent to the last known address of record may be sent by the department to the licensee by electronic means if the licensee has provided an e-mail address to the department.
