Any certification may be revoked or suspended: (1) For any material false statement in the application or in the supplemental or additional statements of fact or studies required of the applicant when a true answer would have warranted the board’s refusal to recommend a certification in the first instance. (2) For failure to comply with the terms or conditions of the certification. (3) For violation of the provisions of this act or regulations or orders issued hereunder.
