Establishment and maintenance of a central registry

The department shall establish and maintain a central registry of persons who have moderate-to-severe brain or spinal cord injuries. (1) Every public health agency, private health agency, public social agency, private social agency, and attending physician shall report to the division within 5 days after identification or diagnosis of any person who has a moderate-to-severe brain or spinal cord injury. The consent of such person shall not be required. (2) The report shall contain the name, age, residence, and type of disability of the individual and such additional information as may be deemed necessary by the department.
