(a) When the State assumes the custody of a live born infant of unknown parentage, an officer of the Department of Services for Children, Youth and Their Families shall report on a form and in a manner prescribed by the State Registrar within 5 days to the Office of Vital Statistics the following information: (1) The date and place of finding; (2) Sex, race and approximate age of child; (3) Name and address of the person or institution with whom the child has been placed for care; (4) Name given to the child by the custodian of the child; (5) Other data required by the State Registrar. (b) The place where the child was found shall be known as the place of birth and the date of birth shall be determined by approximation. (c) The report shall constitute the certificate of birth. (d) If the child is identified and a regular certificate of birth is found or obtained, the report shall be sealed and filed in the Office of Vital Statistics and may be opened only by court order.
