As used in this subchapter: (1) "Health-care practitioner" means as defined in § 701 of this title. (2) "Health-care professional" means as defined in § 701 of this title. (3) "Informed consent" means the signing of a consent form by the patient or a person authorized to make health-care decision on behalf of the patient that satisfies all of the following conditions: a. Is written in plain language. b. Is dated. c. Includes a description of the procedure to be performed. d. States if there will be a student or resident that will perform or be present at the patient's examination.
